Southwestern University Transcript Request - SU uses the National Student Clearinghouse to fulfill all transcript requests. Please follow the link to the right to order your transcript online. Link
Verification of Enrollment (for students - Common verification requests include verification of full- or part-time status, dates of attendance, degree completion, GPA, credit hours, and/or declared major. Other types of information can be verified upon request. Please refer to these instructions  for further guidance. NOTE: Southwestern can only verify enrollment for the current semester or upcoming semester(s) in which you have already registered for classes. Download
Degree Verifications (employers and other agencies) - Southwestern University utilizes the degree verification services from the National Student Clearinghouse.   Link

Parent Access to Student Information (FERPA) - In order for parents/guardians to view grades online, the student will need to grant proxy access through the Self Service website.  Once access is provided, parents/guardians will have the ability to view each semester’s grades and the cumulative GPA.  Proxy access from a student is effective until rescinded by the student


Request and Authorization to Disclose Information (FERPA) - Through this form, the student can authorize the release of specific academic information from a faculty or staff member to the listed person(s).  This disclosure of information is limited to those items authorized on the form, and the release expires at the end of the academic year.

Independent Project - Before any Independent Study, Tutorial, Research Project, or Honors Project course may be added to a student’s schedule, a syllabus with bibliography and signature form must be completed.  This form is now electronic and initiated in the Office of the Registrar.
